Does anyone really use it and if yes how have you got it set out ❓

What I don't like about FS9 is that failures can't occur without you to set them up.
No, I pretty much never use them.

I agree. I want random failures. On a side note, some of the payware planes like the Level-D do have random failures. Not sure about the rest though.
